13+ Music Award Application Form 2

This form is only to be completed if your son missed the registration deadline in Year 6 and is making a late registration alongside a 13+ Music Award application. 

As part of this registration form, you will need to upload the following documents as scanned copies:

• Your child’s full birth certificate showing parental details
• Your child’s passport
• A recent photograph of your child
• Any applicable court orders e.g. parental responsibility, residence, contact etc
• Marksheet for most recent exam taken on 1st study instrument, as well as 2nd and 3rd study instrument(s) if applicable
• Certificate for most recent music theory exam taken (if applicable)
